Free dose log sheets

Ten free, print-friendly injection log sheets: weekly, two-week, monthly and per-cycle. A4, one page each. No sign-up.

Paper records for dose events

These free printable shot logs give you a simple place to write the date, time, compound or label, amount and unit, method, site, and notes. Use the sheet as a record of what happened, not as a prescription or reminder to take anything. The collection includes weekly, two-week, monthly, cycle, landscape, portrait, minimal, and lightly styled layouts so you can choose a page that fits your routine.

Choosing a template

Classic and Clean provide a weekly grid; Two-Weeks gives a longer view; Monthly provides 26 rows; and Cycle leaves room for schedule context. Tall is portrait-oriented, while Paper, Forest, Sky, and Minimal offer different visual treatments. Each PDF is designed for one-page A4 printing. Download an individual page when you want a focused record, or use the combined PDF and ZIP links below to keep the set together.

What to record carefully

Write units exactly as shown, distinguish planned events from completed events, and note the site if rotation matters to your own record. You can add inventory or expiration notes in the margin, but the sheet cannot verify a label, sterility, potency, storage condition, or identity. Keep completed pages private and dispose of them securely if they contain personal information.

Digital alternative and limitations

If you prefer searchable records, the PepLedger tracker can organize doses, cycles, reminders, inventory, body metrics, and selected bloodwork on iPhone. The half-life guide explains why any active-system estimate is only a model. Paper logs are informational tools, not medical advice, and they do not recommend an amount or schedule.
